You will need to submit a few documents in order to raise the claim for reimbursement. You might require discharge summary, original bills, a photocopy of the Apollo Munich Health Card and other documents attached to duly filled claim form. Apollo Munich's telephonic or online assistance would further guide you through all the forms and 'proof of claim' required.

Once all these is submitted at the Third-Party-Administrator (TPA), Apollo Munich will issue you the cheque of due amount against your claim in specified number of working days, subject to the terms and conditions.
